All calls to Medical Imaging are answered by a "live" voice, not a recording, unless all of our lines are busy. We request that you or your office provide us with the following information.

1. In order to make your patient's visit as convenient and "hassle" free as possible, we request that your office provide us with the following information.
    
   

   	Patient's insurance information, person carrying insurance (if patient is a minor or this is a worker's compensation situation)
   	Type of procedure being requested and pertinent diagnosis
   	Patient's name and date of birth
   	Patient's social security number
   	Patient's home phone number and work phone number
   	Primary care doctor (especially important if the patient has HMO insurance) and ordering physician

    
   
2. Previous films should be brought if pertinent. (For example, mammogram films taken at another facility, lumbar spine films [if having a CT of the lumbar spine, etc.] should be brought.)
    
3. Patients should arrive at Medical Imaging 15 minutes before their scheduled examination, unless otherwise noted.
    
4. Although patients with an appointment are given first priority, we are always willing to work patients into the schedule whenever possible.
